Alzheimer's Society NEL

Alzheimer’s Society is the UK’s leading dementia charity. We give vital support to those who need it, fund groundbreaking research, and campaign to improve experiences. It will take a society to beat dementia.

Our Local Services:

  • Pre-diagnostic support service – for anyone experiencing mental health concerns about their memory, including those who are within the Navigo Memory Assessment Service.
  • Dementia Support Service – Personalised information, guidance, and ongoing support for individuals diagnosed with dementia, as well as for their families and carers.
  • Side By Side (SbS) Service – Through our dedicated coordinator, volunteers are matched with people living with dementia to provide one-to-one companionship. This service helps individuals stay active, enjoy local activities, reduce loneliness, and remain socially connected.
  • Memory Cafes – We host three welcoming cafés each month in Cleethorpes, Immingham, and New Waltham. These friendly, informal groups offer a safe space for people living with dementia and their carers to meet, socialise, and build supportive friendships.
  • Weekly Social groups – Delivered in partnership with Creating Positive Opportunity, these groups are open to anyone with a dementia diagnosis or memory concerns, along with their carers.
  • Carers Information and Support Programme (CrISP) A series of group sessions providing practical guidance and advice on all aspects of caring for someone with dementia.
  • Dementia Advisory Group – An opportunity for people with lived experience of dementia to share their insights and help shape and improve local services.
  • Dementia Friends program An initiative aimed at raising awareness and understanding of dementia, encouraging communities to take small but meaningful actions to support those affected.

Additional Support Services

Alongside our local offer, Alzheimer’s Society also provides companionship services to help reduce loneliness and support emotional & mental wellbeing:

  • Regular, friendly telephone calls with a dedicated volunteer
  • Access to an online Dementia Support Forum, where you can connect with others, share experiences, and join weekly Zoom sessions
  • Friendly online spaces such as the “Tea Room” for hobbies, interests, and informal conversation

Assisted Living Centre

The centre provides advice and information for people who require equipment and support at home to be able to maintain their independence.  People can get advice on a range of equipment available to help them at home including wheelchairs, hoists, walking frames and even home adaptations including stair lifts.

The community equipment store is based in the centre, where many of these items are loaned out from. It also has a demonstration area featuring a kitchen, bedroom, bathroom and lounge, where people can see how equipment that could help them at home works. There are also clinic rooms, a wheelchair services workshop and a podiatry lab on-site as well.

Community Diagnostics Centre

The Phlebotomy Service operates within the Grimsby Community Diagnostic Centre (CDC) located in the Freshney Place shopping centre, Grimsby, and can be used for 16 yrs+.

Patients are encouraged to use the web link and QR code in practice to book their own appointment. Blood forms should be given to the patient to take with them to their appointment at the CDC.

The clinician is required to provide the patient with a request form that they can bring to their appointment or generate a WebV request so that the reception staff at the CDC are able to print this for the patient on their arrival.

If patients are not digitally enabled, or are not confident booking their appointment online, Care Navigators at the practice can assist with the process or book on behalf of the patient.

This process is not a replacement for requesting blood tests via Web V. It is simply an access route for booking an appointment. Web V should still be used to request tests, and blood forms must be given to the patient to take with them to their appointment.

Where secondary care has requested bloods following an outpatient appointment, the patient will be given a blood form at their OP appointment.
