Well Together

*This is not a self-Referral Service*

Referrals to the Well Together service can be made by Navigo mental health services and With You.

Criteria: Well Together is a dual diagnosis service for people in North East Lincolnshire with comorbid mental health and substance misuse issues.

Essential Criteria:

  • Age 18 years or older
  • Alcohol Audit score over 15
  • DAST-10 over 3 (or recent illicit drug overdose)
  • Moderate to severe mental health issues (not exclusive to Severe Mental Illness)

A joint project between Navigo and With You, Well Together was created to ensure that no one falls through the cracks when facing complex, dual diagnosis challenges.

Well Together give clients access to intensive, short-term care — up to three appointments a week for eight weeks. The service is flexible, person-centred and built around each individual’s needs. Well Together offers:

  • mental health and substance misuse assessments
  • a small, consistent team to build trust and reduce repetition
  • medication reviews and prescribing
  • emotional support and practical help, such as housing and benefits
  • a clear plan for transition to long-term care

Early intervention helps reduce crisis presentations, hospital admissions and disengagement from healthcare services.

Each person referred to Well Together is assigned a Key Worker and Support Lead who coordinate their care.

Appointments take place wherever a client is most comfortable. This could be at a Navigo or With You building, a community venue, a GP practice or even in their own home.

Initial access to support from the community nursing team is through GP referral

Initial access to support from the community nursing team is through GP referral

The Community Nursing service provide home, residential care or community based nursing care, treatment and support to people registered with a GP practice in North East Lincolnshire who are housebound or have severe mobility problems which means they are unable to attend the surgery for treatment.

The Community Nursing service work in partnership with the local GP Primary Care Network’s (PCN’s) delivering care and support to people requiring on-going nursing care. They are based within several of the GP Practices throughout North East Lincolnshire.

The service is available across GP practices in North East Lincolnshire. Referrals are subject to an assessment by a health or social care professional.

Monday – Friday, 8am – 5pm

Weekends and Bank Holidays – 8.30am – 12.30pm
