Criteria: North East Lincolnshire residents aged 18 years and over.
Free support – You’ll be allocated a Wellbeing Worker who will
give you the time and space to talk and support you with:
• Low mood
• Anxiety
• Stress management
• Weight management
• Daily routines or finding focus in life.
• Confidence & self-esteem
• Healthy eating & physical activity
• Stop smoking.
Or any wider wellbeing challenges that can affect emotional & physical wellbeing, such as relationship issues.

Thrive is a social prescribing project that aims to improve your health and wellbeing by addressing practical, social, and emotional needs. Instead of relying solely on traditional medical treatments, it connects you to local activities, groups, and services that support your overall wellbeing.

• Practical Assistance Services
• Lifestyle and Health Management Programmes
The goal of Thrive is to empower you to take control of your health by engaging with resources that promote a healthier and happier lifestyle. It’s facilitated by working with a link worker over a 12 month period who will help you identify your needs and connect you to appropriate community support.

Criteria: Looking after someone over the age of 60 or with a learning or physical disability. Providing emotional and practical support.

Wellbeing – Medical Professional Initiated Referrals
ABL Health
Criteria: Referral to tier 3 weight management services must meet the clinical criteria set out below:
Be over 18 years and over and registered with a HNY GP and referred by a GP
Have a BMI ≥35kg/m2 with any one of the comorbidities listed:
• Hypertension
• Hyperinsulinemia
• Dyslipidaemia
• Type 2 diabetes
• Psychosocial dysfunction
• Exacerbation of conditions such as asthma
or a BMI ≥40kg/m2 without comorbidities
These BMIs are reduced by 2.5kg/m2 for people from a South-Asian, Chinese, Black African or Caribbean family background.
We are a team of healthcare professionals providing bespoke support to clients.
This includes realistic goal setting, no strict diets or exercise routines and an emphasis on long-lasting lifestyle changes aimed at maintaining a healthy weight after the programme.
What We Offer
• 12-Month Lifestyle/Weight Management Support Program
Our core offering is a 12-month Lifestyle/Weight Management program that includes:
o 11 bi-weekly sessions focused on sustainable lifestyle behaviour changes, followed by (Offered as a group session or 1-1 support)
o 6 monthly sessions for continued support and guidance.
We deliver our sessions in a blended approach, face-to-face, telephone and virtual formats, ensuring flexibility for all clients.
Holistic Health Assessment
We begin with a comprehensive health assessment known as a My Story, taking a holistic approach to understand each client’s physical and emotional health needs understanding their unique journey to ABL Health.
Pathways and Support Offered
We provide several tailored pathways based on individual needs, including:
• Bariatric Pathway
For patients who meet the eligibility criteria, we offer support through a Bariatric Pathway, guiding them through the process and preparation for bariatric surgery if appropriate. Clients are encouraged to develop their knowledge by attending bi-monthly webinars with Salford Royal/Castle Hill.
•
Nutritional Education, Physical Activity and Overall Wellbeing
• The service offers free nutritional and physical activity support and advice, and consider other behavioural and psychological aspects, and life circumstances linked to weight gain and poor health. We also explore alternate diet pathways, such as:
o Low Carb Healthy Fats (LCHF)
o Intermittent Fasting (IF)
• Therapeutic Support
Recognising the emotional aspects of weight management, we offer both 1-to-1 and group therapy sessions focusing on emotional eating and behavioural change.
• Pharmaceutical Pathway
We offer support with Orlistat information sessions.
