Community Services

Community Urgent Eye Service (CUES)

Suitable for all ages, must be registered with a local GP. Children under  must be accompanied by an adult.

Symptoms that can be treated:

  • Red or painful eye or eyelids
  • Recently occurring flashes and floaters
  • Recent and sudden loss of vision
  • Foreign body in the eye
This is an NHS service provided by accredited optometrists (also known as opticians).

Please note, this service is not an eye test.

Conditions NOT covered by this service:
Major eye conditions that are being regularly monitored by an optometrist or hospital eye service, such as cataracts, diabetic retinopathy or glaucoma.

In this case, the patient should consult their usual optometrist or hospital eye service directly for advice in the usual way.

District Nurses – CPG

Available Monday – Friday, 8am – 5pm / Saturday & Sunday, 8.30am – 12.30pm / Bank Holiday, 8.30am – 12.30pm.

The out of hours team is available 7 days a week, 24 hrs a day.

The district nursing team provide a range of interventions including:

  • Assessment of health and social care needs, formulating an agreed care plan
  • End of life care
  • Chronic and acute wound care
  • Education in the prevention of pressure ulcers and provision of equipment
  • Continence assessment and advice
  • Venepuncture for management of patient’s long term conditions
  • Support and advice for patients and carers in self-management of their care
The service is available across GP practices across North East Lincolnshire. Referrals are subject to an assessment by a health or social care professional.

District Nurses – Freshney Pelham

Criteria: Patients must be registered with one of the below three practices within the Freshney Pelham PCN.

We provide a comprehensive range of Community Nursing Care Services with the needs of our patients and their families guiding our nursing care.

These services include:

  • Holistic assessment of health needs
  • Assessment, planning, implementation and evaluation of patient needs in their own home
  • Wound care assessment, for example, surgical, uclers, tears and pressure wounds
  • Health promotion health checks and immunisation programmes within the patients home
  • Palliative and end of life care and associated symptom management
  • Promotion of self-care whenever possible
  • Administration of treatment using specialist nursing equipment and medication
  • Assessment and prescribing of appropriate equipment to facilitate care in the patients home and manage risks to prevent complications

Dental – Emergency Dental Services

Patients registered with a dentist should contact their dentist for emergency dental treatment.

Patients without a regular dentist, should contact NHS 111 to source an emergency appointment.

Ear Syringing/irrigation

Ear syringing is no longer a core service provided by a GP practices.

Patients can be directed to local pharmacies under the Minor Ailments Scheme or Community Pharmacy First for advice on treating ear wax build up.

In cases where patients are experiencing hearing loss they may require an appointment at the practice to be referred to audiology/local microsuction service (dependant on eligibility).

Inclusion Criteria for miscrosuction referral:

  • Suspected/confirmed history of ear drum perforation
  • Recurrent/recent Otitis Externa
  • Previous mastoid or ear surgery
  • Gromments in place
  • Complete unilateral deafness in contralateral ear
  • Otalgia or otorrhoea
  • A requirement to view tympanic membrane
  • Impacted wax which has not cleared with conservative management.

Exclusion Criteria for microsuction referral:

  • Children aged 5 and under
  • Removal of simple ear wax
  • Previous history of contra-indications following miscrosuction
  • History of dizziness
  • Having a condition which can cause sudden movements – e.g. Parkinson’s
  • Overly sensitive to loud noises

North East Lincolnshire Council – School Nurses

The School Nursing service is available to children from the age of 4.5 yrs, young people and their families. We work in partnership with GP’s, schools, family hub practitioners and various other professionals to meet identified needs and provide the best possible care.

  • Relationships and sexual health

North East Lincolnshire Council – Health Visitors

The Health Visiting service is available to all families in North East Lincolnshire who are expecting a baby, have a new born or a child up to the age of 5.

Information on Health Visitor appointments

  • Antenatal appointment – offered to you from 28 weeks pregnant, you’ll meet your health visitor who will talk about our service and preparing for your baby’s arrival
  • New birth visit – your health visitor will come to your home when your baby is 10 – 14 days old. You can ask any questions and talk about your first weeks with your new baby.
  • Postnatal contact – your health visitor will come again 6 – 8 weeks after birth, where they will review your baby’s development, growth and overall wellbeing
  • Health review – appointments at 9 – 12 months and 2 – 2.5 yrs old to check up on your child’s development, growth and overall wellbeing.

Podiatry – NLAG Self Request for Podiatry Assessment

This service is for people aged 16 years of age and over with podiatry problems and registered with a local GP practice.

This self-referral route allows patients to access Podiatry Services quickly, without needing to see a GP or health professional first.

Do not use self-referral:

  • If your GP is out of our geographical area
  • If you have received treatment from the podiatry service for the same condition in the last 6 months
  • If you require routine nail cutting and skin care (corns and callus) and you don’t have an underlying long term medical condition
  • If you require advise for fungal nails or verrucae
  • If you are looking for a home visit and are not bed bound. We provide a very limited home visit service and would support you to attend one of our clinics.

Patients who are able to attend and with non-complex issues can access the drop-in clinic at the Assisted Living Centre by telephoning Single Point of Access or by drop in at the clinic (although no guarantee of same day/time appointment).

Some items of equipment at the Assisted Living Centre can be offered for self-purchase without assessment or if the individual doesn’t meet criteria to provide.

Where requesting an assessment, individuals can be assessed with a range of equipment on site.

This functional assessment, carried out at the clinic can lead to simple provision or for more complex needs an onward referral can be made to Community Occupational Therapy or Physiotherapy on behalf of the individual.

Self-referral can be accessed by phoning the admin team who support patients to gain the details needed on the referral form before passing onto clinicians for triage and then any necessary intervention can be arranged.

All referrals are triaged to ensure they meet criteria and should progress to assessment with a clinician. In addition, where there are complex issues raised in the requests other clinicians involved may be contact for corroborating information or for advice.

Growing Well Together – Navigo Parent-Infant Relationship Service (Part of the Start for Life programme)

The service is for for pregnant parents/caregivers and their babies (from conception to two years old), who would like support with building a relationship with their baby.

As a service we are committed to supporting dads, LGBTQ+ parents/caregivers, and families that might find traditional services more difficult to access.

We work flexibly with the caregiver and baby and meet them where they feel most comfortable, which might be their home, a local community centre, a family hub, or at our case in Freshney Green Primary Care Centre.

The support might be delivered in a group, one to one r through a trusted professional already working with the family. In addition to direct support with caregivers and babies, we also offer one off and ongoing support and advice to professionals working with the family. Please don’t hesitate to pick up the phone and speak to us!
