Team Around the Family
Delivered across North East Lincolnshire |
Criteria: Families with children aged 0-18.
Referral into this service should be through a professional who knows the family (e.g. nursery, school, midwife, health visitor), and where there is a need for more structured and targeted help to prevent needs from escalating, and to support them through times that may be challenging. Parental consent is needed to establish a Team Around the Family.
Details of what the service provides and any appropriate relevant information · A currently involved professional can work with the family to carry out a multi-agency Early Help Assessment to identify the children’s needs and develop a plan of support. · Following assessment, a Lead Professional should be agreed with the family and a family-led, multi-agency plan developed with the family and professionals supporting them. · The Team Around the Family can meet every 8 weeks to review the goals that have been set and discuss next steps. · Goals can focus on a variety of needs, including parental conflict, parenting support, welfare and debt management, housing, early intervention on mental health and more.

The Team Around the Family approach uses families’ existing network of professional and family support, rather than necessarily bringing on board a new professional. Criteria: Families with children aged 0-18
Service for when children’s needs are such that a more structured support plan may be required to co-ordinate the help needed to achieve agreed outcomes. Parental consent is needed to refer.
Families may already be open to a Team Around the Family led by other professionals supporting them.
Details of what the service provides and any appropriate relevant information · Where families are not already working with a professional, Family Help can work with the family to carry out a multi-agency Early Help Assessment to identify the children’s needs and develop a plan of support. · If needed, Family Help will lead a Team Around the Family and meet every 8 weeks to review the goals that have been set and discuss next steps. · Family Help can deliver one to one packages of support with parents and children, around a variety of subjects, including: o parenting support for all ages, o support for young carers, o support around home conditions, o support around the impact of substance use, o support around child exploitation, o support for children going missing, o support around healthy relationships and more.
Information for Professionals: Some of our specific programmes include: · Respect one to one programme for Domestic Abuse perpetrators · DART (Domestic Abuse Recovering Together) group programme for mothers and children aged 7-14 · Solihull Parenting Support programmes for parents with children aged 0-19 · Stepping Stones Parenting Support programme for parents with children with special education needs aged 0-11 · Girls Group – group intervention for older girls covering healthy relationships |

Family Hubs and Start for Life |
Criteria: Family Hubs 0 to 19 years old (25 years old for a young person with a disability or special need).
Criteria: Start for Life 0-2years (24months)
Family Hubs Supporting families to give their Child the best start for life. Our Family hubs provide a setting for services to be connected and work together, to help families access help and support at a local level. From thinking about having a baby through to the age of 19 years old (25 years old for a young person with a disability or special need).
Families will be able to access the right support at the right time in the right place: · Becoming a parent · Easy, local access to registering your baby’s birth · Infant Feeding · Understanding your development child · Parenting and co-parenting · Supporting your mental health needs · Raising aspirations for families · Signposting and helping parents to access services that can help
Start for Life Start For Life includes what is available to support your family in the first 1,001 days. This is from conception to 2 years old (24 months).
The 1,001 critical days is when the foundations of the brain’s architecture are built. · Midwifery Services preparing for your baby- Anti-natal classes, health visiting, pre-birth courses, maternal support services · Birth – health visiting, infant feeding, Mental Health Support · 0-3 months small groups to support bonding and early development; baby Massage, baby Rhyme time · 3-24 months Health check ins, Groups to meet other parents, learn about your child’s development and receive support After the first 1001 days |
24 weeks + continued access to play sessions, health checks and access to support.
Our Family hubs are friendly and Welcoming environments to play with your child and learn fun activities to help them develop and mix with other children. We provide a setting for services to be connected and work together, to help families access help and support at a local level. Our goal is to also build capacity to connect with our communities and give parents opportunities to make friends and develop support networks. |

NLaG – Midwives/Antenatal |
If a patient has done a positive pregnancy test the next step is to contact a midwife as soon as possible, preferably before eight weeks into the pregnancy. Patients can contact a local midwife direct, and don’t need to see a GP first.
Work in partnership with families throughout pregnancy, birth and afterwards to offer all the services and support needed.

Growing Well Together – Navigo Parent-Infant Relationship Service (PaIRS) |
Criteria: Pregnant parents/caregivers and their babies (from conception to two years old). Support for parents and care givers who are feeling overwhelmed, vulnerable or stressed to build strong, nurturing relationships with their babies. As a service we committed to supporting dads, LGBTQ+ parents/caregivers, and families that might find traditional services more difficult to access. We work flexibly with the caregiver and baby and meet them where they feel most comfortable, which might be their home, a local community centre, a family hub, or at our base in Freshney Green Primary Care Centre. The support might be delivered in a group, one to one or through a trusted professional already working with the family. In additional to direct support with caregivers and babies, we also offer one off and ongoing support and advice to professionals working with the family. Please don’t hesitate to pick up the phone and speak to us! Please read the following before proceeding with your referral: · This service operates Monday to Friday 9am – 5pm. · We ask that you ring to discuss your referral prior to send it by calling 01472 252 570 and booking in a suitable time with a member of our team to talk through why you think this might be a helpful service to receive support from. · Please complete all fields of the form as it may delay the processing if information is missing.
