The Continence Service provides a range of services including:
• Assessments, development and ongoing review of treatment or management plans for your continence problem
• Tips to promote a healthy lifestyle to improve or control symptoms- i.e. changes to diet or fluid intake
• Pelvic floor exercise programme
• Medication advice
• Support bladder and bowel training programmes
• Provide continence products where applicable
• Advice and support with catheter problems
• Telephone support for patients and healthcare professionals

Continence Symptoms

There are different symptoms and reasons for continence problems including:
• Stress incontinence – Urinary leakage when you cough, laugh or sneeze
• Urge incontinence – Needing to rush to get to the toilet and sometimes leaking before you get there
• Nocturia – Waking up more than a couple of times in the night to pass urine
• Functional incontinence – Having a condition or disability that stops being able to remove your clothes quickly or get to the toilet in time
• Bowel Incontinence – Difficulty in controlling bowel movements

District Nurses – CPG

Criteria: Patients must be registered with a GP in North East Lincolnshire.

Initial access to support from the community nursing team is through GP referral.

The district nursing team provide a range of interventions including:

• Assessment of health and social care needs, formulating an agreed care plan
• End of life care
• Chronic and acute wound care
• Education in the prevention of pressure ulcers and provision of equipment.
• Continence assessment and advice
• Venepuncture for management of patient’s long term conditions.
• Support and advice for patients and carers in self-management of their care.
The service is available across GP practices across North East Lincolnshire. Referrals are subject to an assessment by a health or social care professional.

We provide a comprehensive range of Community Nursing Care Services with the needs of our patients and their families guiding our nursing care.

These services include:

• Holistic assessment of health needs
• Assessment, planning, implementation and evaluation of patient needs in their own home
• Wound care assessment, for example, surgical, ulcers, tears and pressure wounds
• Health promotion health checks and immunisation programmes within the patients home
• Palliative and end of life care and associated symptom management
• Promotion of self-care whenever possible
• Administration of treatment using specialist nursing equipment and medication

Podiatry – NLAG Self Request for Podiatry Assessment

This service is for people aged 16 years of age and over with podiatry
problems and registered with a local GP practice.

This self-referral route allows patients to access Podiatry Services quickly, without needing to see a GP or health professional first.

Do not use Self-Referral
• If your GP is out of our geographical area
• If you have received treatment from the podiatry service for the same condition in the last 6 months
• If you require routine nail cutting and skin care (corns and callus) and you don’t have an underlying long term medical condition.
• If you require advise for fungal nails or verrucae
• If you are looking for a home visit and are not bed bound. We provide a very limited home visit service and would support you to attend one of our clinics.

NLaG – ADL (Activities of Daily Living) equipment

Patients who can attend and with non-complex issues can access the drop-in clinic at the Assisted Living Centre by telephoning SPA or by drop in at the clinic (although no guarantee of same day/time appointment).

Some items of equipment at the Assisted Living Centre can be offered for self-purchase without assessment or if the individual doesn’t meet criteria to provide.

Where requesting an assessment, individuals can be assessed with a range of equipment on site.

This functional assessment, carried out at the clinic, can lead to simple provision or for more complex needs an onward referral can be made to Community Occupational Therapy or Physiotherapy on behalf of the individual.

NLaG – Wheelchairs

Self-referral can be accessed by phoning the admin team who support patients to gain the details needed on the referral form before passing onto clinicians for triage and then any necessary intervention can be arranged.

All referrals are triaged to ensure they meet criteria and should progress to assessment with a clinician. In addition, where there are complex issues raised in the requests other clinicians involved may be contacted for corroborating information or for advice.

HOPE Specialist Falls Service

Criteria: older people aged 65 or over and living in their own home/group dwellings. Self-referral or referral by a health professional.

People under the age of 65, or over 65 but living in residential/care homes should be referred to the Community Therapy team.

The Falls Service offers several options:

Fallstop – a one-off group session, which involves advice and education about:

• What can cause a fall and how to prevent them
• The importance of staying active
• Keeping walking aids safe
• Overcoming difficulties with accessing transport
• Getting out and about

Falls Rehabilitation Programme – an eight week, twice weekly programme of exercise and education, which you will attend as part of a group of up to ten patients. The programme is tailored to your needs and abilities, so no matter how little you think you can do, we can design a programme to help.

Home Rehab – a six-week tailored programme of exercise, delivered in your own home. The programme is flexible to work around you and any appointments or other commitments you have. It will utilise the home environment and can also incorporate outdoor activity practice, such as walking outside or functional tasks like hanging out the washing and gardening.

Virtual Falls Rehab – an eight week, twice weekly programme of exercise and education, similar to our Falls Rehabilitation programme, but which can be delivered to you in your own home.

Single Point of Access (SPA)

Criteria: Residents of North East Lincolnshire

Option 1: For Health Related Matters
You should call Single Point of Access if:
• You need medical help fast, but it is not a 999 emergency
• You think you need to go to accident and emergency or another NHS urgent care service
• You do not know who to call for medical help or you do not have a GP to call
• You require health information or reassurance about what to do next
• For less urgent health needs, you should still contact your GP in the usual way.

Option 2: For Adult Social Care or Therapy Enquiries
You can contact the Single Point of Access if you or someone you care for needs information or advice about adult social care. This includes:

• Support at home
• Support for unpaid carers of adults including carers’ assessments
• Child to adult transition support
• Community occupational therapy referrals
• Sensory impairment (visual or hearing impairment) support

Criteria: Patients/Referrals need to be over the age of 59, experiencing social isolation and loneliness and live in Grimsby.

Our project provides a Social Prescribing Community Link worker to support the individual for up to 12 months.
• Offering Face to face support, goal setting and signposting
• Working through issues surrounding loneliness and social isolation
• Early intervention to prevent a decline in physical and mental health.
• Referring into all relevant support services, Care Alarms, Occupational Therapy assessments, Benefits Advice referral and Fire Safety in the home.
• Manage and health conditions via various NHS services
• Support into a regular engagement of Groups Clubs and Activities.
