Supporting adults with learning disabilities and their carers improve their health and wellbeing.
The CLDT is a multi-disciplinary team that provides specialist health services to support these additional needs. The team consists of:

A range of therapeutic services and activities for people with disabilities or complex needs.
Based at Cromwell Road Day Centre, Grimsby, day services provide a wide range of activities and support for adults with disabilities or complex needs.
Day services provide the support, activities and stimulation to help this transition as a young person moves into the next phase of their life.

Linkage Choices – Adult Day Services

Linkage Community Trust, commonly referred to as Linkage, is a registered charity that supports people with learning disabilities and autism.
Linkage Choices is a day service for adults with learning difficulties and disabilities aged 16+.
Open Monday – Friday, with a range of evening and weekend activities available.
Individuals can attend Linkage Choices on a flexible basis, to suit their individual needs, which could be from half a day to five full days, or just one of the evening or weekend activities.
We focus our support delivery on four main areas: health and wellbeing; employability, independence; and friendship, relationships and community.

The Adult Autism Service

The Adult Autism Service is able to offer a full range of services for Autistic Adults, including specialist Autism Diagnostic Assessment for adults who, for a variety of reasons, may not have been diagnosed as a child, and support to enable autistic adults to live the life they want.

We have an open referral process. Anyone can refer to the service by completing a referral form and returning it to the Adult Autism Service.
Carers Wellbeing Team – CPG

Criteria: Looking after someone over the age of 60 or with a learning or physical disability. Providing emotional and practical support.
For example, support with attending meetings, completing paperwork, and signposting them to other organisations that provide further help. We are there to provide a listening ear to hear our carer’s struggles and look at what they may need at that time to make their lives a little easier
Special Educational Needs and Disabilities (SEND) North East Lincolnshire
The Local Offer in North East Lincolnshire the place that provides and directs you to information that will support children and young people with Special Educational Needs and Disabilities (SEND).
It helps parents, children and young people aged 0 – 25 years find information about specialist and targeted services and activities in North East Lincolnshire.

It’s impartial and we give confidential information, advice and support in relation to special educational needs and disabilities (SEND) and related health and social care matters.

NELPCF is a collective voice for parent carers of children and young people with Special Educational Needs and Disabilities (SEND) in North East Lincolnshire. They make sure that parents’ views and experiences influence local services to make things better.

As a parent carer forum, we are not able to provide individual advice and support.

We run a free helpline and offer a Listening Ear support service We produce a range of email newsletter and parent guides available to download or order.
Local offices & family events
We have local offices in Northern Ireland Scotland and Wales and across London.
Aside from providing local advice, our regional teams organise coffee mornings, family fun days and outings, bringing families together for friendship and support.

Employability Services supports local people into sustainable employment across North East Lincolnshire aged 16 years up to retirement.
Supported employment is a flexible service for people with health conditions, learning or physical disabilities and/or neurodiverse conditions.
Following an initial meeting and assessment with an experienced team member, we will work together to decide on the best pathway for you which could include one of the following:
- Paid opportunities
- Voluntary work placements
- Supported internships
