Criteria: Well Together is a dual diagnosis service for people in North East Lincolnshire with comorbid mental health and substance misuse issues.
Essential Criteria:
- Age 18 years or older
- Alcohol Audit score over 15
- DAST-10 over 3 (or recent illicit drug overdose)
- Moderate to severe mental health issues (not exclusive to Severe Mental Illness)
A joint project between Navigo and With You, Well Together was created to ensure that no one falls through the cracks when facing complex, dual diagnosis challenges.
Well Together give clients access to intensive, short-term care — up to three appointments a week for eight weeks. The service is flexible, person-centred and built around each individual’s needs. Well Together offers:
- mental health and substance misuse assessments
- a small, consistent team to build trust and reduce repetition
- medication reviews and prescribing
- emotional support and practical help, such as housing and benefits
- a clear plan for transition to long-term care
Early intervention helps reduce crisis presentations, hospital admissions and disengagement from healthcare services.
Each person referred to Well Together is assigned a Key Worker and Support Lead who coordinate their care.
Appointments take place wherever a client is most comfortable. This could be at a Navigo or With You building, a community venue, a GP practice or even in their own home.
